Why ingestion failed for a Document or a run (1.53.0; knowledge-bases.md section 3): unsupported_format, parse_timeout and too_large from the parse stage (the names the document gives); fetch_failed (the bytes could not be read after the retries of the job); fetch_not_allowed (the egress guard or robots.txt refused the URL); embedding_failed (no Provider of the embedding space answered after the retries). From 1.54.0: classification_unavailable (in PHI mode the classify stage failed closed, because a detector PHI mode requires, the model-assisted entity stage, could not run; a retry names it, and a task that runs out of attempts fails with it), and retries_exhausted (the task ran out of attempts, 5 a target, and no retry named a code; until 1.54.0 the api recorded fetch_failed there). A task that runs out of attempts fails with the last code a retry named. Codes are added in later minor versions.
