What one evaluation of a Policy gave (orchestration-layer.md 3.8, step 4): matched (its document output an action), not_matched (no action), timeout (the hook budget ran out before it was evaluated; from 1.65.0, its evaluation passed the per-Policy ceiling) or error (the engine failed, or the output was not an action its type accepts).
