Why an evaluation is error or timeout: the engine failed (engine_error), the output was not an action the Policy's type accepts (output_invalid), the hook's wall-clock budget ran out (wall_clock), or (1.48.0) the wrapper refused the context before the engine ran, because a number in it was not finite or beyond the safe-integer range or it nested too deep (context_out_of_bounds), or (1.65.0, ADR 0093) its own evaluation passed the per-Policy ceiling and its result was discarded (evaluation_ceiling); from 1.65.0 no new record carries wall_clock.
