Why a transition was not taken; this short reason is also returned to the model when its proposal is refused. guard_false: the guard returned false. missing_slots: required slots are empty, listed by name. limit_reached: a handoff beyond max_handoffs_per_turn. policy_blocked: a Policy blocked it at on_tool_call or vetoed it at on_state_exit.
